Proof of income (recent pay stubs, benefit letters, or tax forms)
Most recent bank statement (checking/savings accounts)
A copy of your Kansas Medicaid Denial Letter
Last year’s federal income tax return (all pages)
A completed and signed Financial Assistance Application
