At Graham County Hospital, our Chronic Care Management (CCM) program supports patients with two or more chronic conditions such as arthritis, diabetes, heart disease, high blood pressure, depression, and osteoporosis. This Medicare-supported program helps you manage your health through regular check-ins, care coordination, and personalized support between visits.

What’s Included

  • At least 20 minutes per month of dedicated chronic care coordination
  • Personalized care plan created with help from your dedicated care manager
  • Ongoing communication between your pharmacy, specialists, labs, and primary care team
  • Phone check-ins between visits to help you stay on track
  • Goal setting and health education to help you reach your wellness targets
